This form is fast — you mainly just need the venue information and your event details.
Once submitted, most COIs are delivered in 1–3 business days via email.
1) Venue / Certificate Holder
Enter the venue/school/church name and the address (or the certificate holder details provided by the venue).
2) Event Details
Add your event date, location, and a short note about what you’re renting (if known).
3) Submit + Watch Email
After submission, you’ll receive your COI by email (typically 1–3 business days).
Tip:
If your venue has special wording requirements, paste them into the notes section of the form — we’ll match it as closely as possible.
